Double-decker bus crash leaves 3 dead, 23 injured in Ottawa

The city's police chief Charles Bordeleau said the bus driver was arrested at the scene and taken in for questioning. Photos posted on Twitter shows extensive damage to the front of the bus, with the roof of a shelter pushed well into the bus's second deck.

"Please keep victims, witnesses, families and our wonderful first responders in your thoughts". In that case, an investigation found that the cause was a combination of excessive speed, a hard curve before the tracks and driver distraction from video monitors showing the upper deck.
