Series of failures led to deadly Niger ambush-US military report

Contrary to some earlier media reports and eyewitness accounts, the report says none of the USA forces were captured before they were killed, and none of them suffered.

While the mission was underway, the team was ordered by superiors to assist a separate USA team sent to search for Chefou by helicopter.

A summary of the report released Thursday details the need for improved mission planning and approval processes, in addition to a review of weapon requirements and training exercises with US -allied forces, according to The Associated Press.

Killed in the battle were Staff Sgt. Jeremiah W. Johnson, 39; Staff Sgt. Bryan C. Black, 35; Staff Sgt. Dustin M. Wright, 29; and Sgt.

Waldhauser, the commander of U.S. Africa Command, and Army Maj.

Mattis also has directed U.S. Special Operations Command to review training, operating procedures, operational-level planning and other relevant factors, and he has directed the undersecretary of defense for personnel and readiness to review DOD policies that adversely affect units' cohesion and lethality.

"The investigation identifies individual, organizational and institutional failures and deficiencies that contributed to the tragic events of 4 October 2017.no single failure or deficiency was the sole reason for the events", an eight-page summary of the report says. The document "contributed to a general lack of situational awareness and command oversight at every echelon".

If those conditions are not met, the US troops will work from fortified command centers to advise African forces on intelligence, logistics, artillery and other aspects of big operations that are important - but not as flashy as front-line combat against a range of groups aligned with al-Qaida or the Islamic State group.
